    Extractions

    When it comes to dental procedures, tooth extraction — or having teeth “pulled” is among patients’ most dreaded prospects. Also referred to as exodontia, tooth extraction involves removing a tooth from its socket in the jaw bone. Before your dentist considers extraction, every effort will be made to try to repair and restore your tooth. However, sometimes it is necessary.

    Reasons for Extraction

    • Severe Tooth Damage/Trauma – Some teeth have such extensive decay and damage (broken or cracked) that repair is not possible. For example, teeth affected by advanced gum (periodontal) disease may need to be pulled. As gum disease worsens, the tooth, supported by less surrounding bone, — often loosens to such an extent that extraction is the only solution.
    • Malpositioned/Nonfunctioning Teeth – To avoid possible complications that may result in an eventual, negative impact on oral health, your dentist may recommend removing teeth that are malaligned and/or essentially useless (teeth that have no opposing teeth to bite against).
    • Orthodontic Treatment – Orthodontic treatment, such as braces, may require the removal of teeth to make needed space for improved teeth alignment.
    • Extra Teeth – Also referred to as supernumerary teeth, extra teeth may block other teeth from erupting.
    • Radiation – Head and neck radiation therapy may require the extraction of teeth in the field of radiation in order to help avoid possible complications, such as infection.
    • Chemotherapy – Chemotherapy weakens the immune system, increasing the risk of tooth infections, heightening the risk of extraction.
    • Organ Transplant – Immunosuppressive medications prescribed after organ transplantation can increase the likelihood of tooth infection. As such, some teeth require removal prior to an organ transplant.

    The 2 types of Extraction

    When you undergo a tooth extraction procedure, your dentist will numb the area with a local anesthetic. You may also receive an anti-anxiety medication or an intravenous sedative. If the dental extraction involves an impacted tooth, the tooth may be broken into pieces before it is removed.

    Pulling teeth falls into two basic categories: simple and surgical. Here’s what to expect from each:

    • Simple Extractions – A simple tooth extraction involves the removal of a tooth that is visible in the mouth. This could mean removing a badly damaged or decayed tooth, or removing teeth prior to getting braces. General dentists can do simple tooth extractions. When you undergo simple tooth extractions, you will receive local anesthesia. In addition, some dental professionals administer anti-anxiety medication or use conscious sedation for simple cases of pulling teeth. In most cases, over-the-counter pain medication is sufficient for pain management after these procedures.
    • Surgical Extractions – A surgical tooth extraction is an operation by an oral surgeon involving removal of teeth that are not visible in the mouth, because they have not come in or because the tooth has broken off. Individuals with special medical conditions may receive general anesthesia when pulling teeth involving surgery. You may also receive prescriptions for pain medication to use immediately after surgical teeth-pulling procedures.

    The Procedure

    Your dentist will explain how to prepare for your procedure. They’ll ask about your dental and medical history. It’s important to let them know about any medical conditions, allergies or recent surgery, as well as any medicines you’re taking.

    Your dentist will discuss with you what will happen before, including any pain you might have. If you’re unsure about anything, ask. No question is too small. Being fully informed will help you feel more at ease and will allow you to give your consent for the procedure to go ahead.

    You’ll usually have your tooth (or teeth) removed under a local anaesthetic. This completely blocks pain from your gums, although you’ll still feel pressure. You’ll stay awake during the procedure, so you’ll be aware of what’s happening. If you’re very anxious about having your tooth removed, it might be possible to have a sedative, which relieves anxiety, makes you feel sleepy and helps you to relax.

    Having a general anaesthetic for an extraction is usually only an option for young children or adults with learning disabilities. However, your dentist may decide it’s right for you if several of your teeth need to be removed, or the extraction is going to be more difficult than usual.

    If you’re going to have a general anaesthetic, your dentist will refer you to a hospital to have your procedure.

    • Once you’re sitting comfortably in a chair, your dentist will inject a local anaesthetic into the area around your tooth or teeth. They’ll wait a few minutes to allow the injection to work and ask you a few questions to see if it’s taking effect.
    • The roots of your tooth sit in a socket (hole) in your gum. Your dentist will widen your tooth socket and gently loosen your tooth before they remove it. Sometimes your dentist may need to put a stitch in the empty socket to help it heal.
    • You’ll feel some pressure in your mouth when you have a tooth removed but it shouldn’t be painful. If you do feel any pain, let your dentist know straightaway.
    1.Aftercare for tooth removal

    Your gum may bleed for a few minutes after you have your tooth taken out. Your dentist will give you a piece of soft padding to bite on to stop the bleeding and you’ll be able to leave the clinic once it’s stopped.

    Before you leave the clinic, your dentist or surgeon will give you advice about looking after your teeth and gums. They may recommend painkillers and an antibacterial mouthwash. They might also prescribe you some antibiotics to reduce your chances of developing an infection.

    If you have had a general anaesthetic or sedative, you’ll need to rest until the effects of the anaesthetic or sedative have worn off. Ask a friend or family member to stay with you for a day or so while the anaesthetic wears off.

    You don’t always need a follow-up appointment after you’ve had a tooth removed. But if you had a complicated procedure, you might need to go back to see your dentist so they can check how you’re healing. You’ll be given a date for this while you’re still in Antalya.

    Most people can go back to their normal routine the same day. Only if you have a more difficult surgical extraction, will it take a few days to recover. See how you feel and follow your dentist’s advice.

    If you had local anaesthetics, it may take a few hours before the feeling comes back into your mouth. Don’t have any hot food or drinks until it comes back otherwise you might burn or scald your mouth. Also take care not to bite your tongue, particularly when you speak, drink or eat. Rest as much as possible and keep your head up to reduce the bleeding.

    Your mouth may feel sore once the anaesthetic wears off. If you need pain relief, you can take over-the-counter painkillers such as paracetamol or ibuprofen.

    Some people find that their pain is worse about three days after the procedure, but then settles down again within a week to 10 days. This is completely normal.

    • Don’t rinse your mouth out for at least 24 hours after a tooth removal. This could disturb any blood clot that has formed, and you may start bleeding again.
    • After 24 hours, rinse gently with a salt water mouthwash (made using salt and hot, but not boiling water), four times a day to keep the area clean.
    • Eat soft food once you first have your tooth removed, so you don’t have to chew much.
    • If your gum bleeds, bite down on a clean pad of material such as a clean handkerchief for at least 15 minutes.
    • Don’t drink alcohol for at least 24 hours and don’t smoke for as long as possible, but at least for the rest of the day.
    • Brush your teeth but keep your toothbrush away from the healing wound, to begin with, brushing closer to it each day. You could try softening your toothbrush in hot water before you brush.

    You may have stitches, depending on which tooth was removed, and why. The stitches will dissolve by themselves within a week to 10 days, so you won’t need to have them removed.

    It’s important to brush these carefully for three to four days after your surgery to stop food getting trapped. But be careful so you don’t dislodge any newly-formed blood clots that may have formed over your empty tooth socket.

      Wisdom Teeth

      Wisdom teeth do not always grow properly aligned with the rest of your flat teeth at the back of the mouth (molars). In fact, having well-aligned wisdom teeth is very rare. Most of the time, they do not grow properly through the gum line due to misalignment, lack of space, and the proximity of adjacent teeth.

      If your wisdom teeth are severely misaligned, a dentist may recommend a wisdom teeth removal procedure. Not to worry, wisdom teeth removal is a simple, inexpensive procedure that offers huge benefits for your oral health.

      When should you have your Wisdom Teeth removed?

      Some of the most common signs that you need to undergo Wisdom Teeth removal are:

      • You have tooth decay (dental cavities, dental caries, a hole in your tooth).
      • You are suffering from an infection on your wisdom tooth.
      • Your wisdom tooth has an existing cyst.

      Things to consider

      The Procedure

      Your dentist or oral surgeon may use one of three types of anesthesia, depending on the expected complexity of the wisdom tooth extraction and your comfort level. The 3 types are local anesthesia, sedation anesthesia and general anesthesia.

      During wisdom tooth extraction, your dentist or oral surgeon:

      • Makes an incision in the gum tissue to expose the tooth and bone
      • Removes bone that blocks access to the tooth root
      • Divides the tooth into sections if it’s easier to remove in pieces
      • Removes the tooth
      • Cleans the site of the removed tooth of any debris from the tooth or bone
      • Stitches the wound closed to promote healing, though this isn’t always necessary
      • Places gauze over the extraction site to control bleeding and to help a blood clot form
      1.Are there any risks involved?

      Most wisdom tooth extractions don’t result in long-term complications. However, removal of impacted wisdom teeth occasionally requires a surgical approach that involves making an incision in the gum tissue and removing bone. Rarely, complications can include:

      • Painful dry socket, or exposure of bone when the post-surgical blood clot is lost from the site of the surgical wound (socket)
      • Infection in the socket from bacteria or trapped food particles
      • Damage to nearby teeth, nerves, jawbone or sinuses

      The differences are:

      • Local anesthesia: Your dentist or oral surgeon administers local anesthesia with one or more injections near the site of each extraction. Before you receive an injection, your dentist or surgeon will likely apply a substance to your gums to numb them. You’re awake during the tooth extraction. Although you’ll feel some pressure and movement, you shouldn’t experience pain.
      • Sedation anesthesia: Your dentist or oral surgeon gives you sedation anesthesia through an intravenous line in your arm. Sedation anesthesia suppresses your consciousness during the procedure. You don’t feel any pain and will have limited memory of the procedure. You’ll also receive local anesthesia to numb your gums.
      • General anesthesia: In special situations, you may be offered general anesthesia. You may inhale medication through your nose or have an IV line in your arm, or both. Then you lose consciousness. Your surgical team closely monitors your medication, breathing, temperature, fluids and blood pressure. You’ll experience no pain and have no memory of the procedure. Local anesthesia is also given to help with postoperative discomfort.

      If you receive sedation anesthesia or general anesthesia, you’re taken to a recovery room after the procedure. If you have local anesthesia, your brief recovery time is likely in the dental chair.

      As you heal from your surgery, follow your dentist’s instructions on:

      • Bleeding: Some oozing of blood may occur the first day after wisdom tooth removal. Try to avoid excessive spitting so that you don’t dislodge the blood clot from the socket. Replace gauze over the extraction site as directed by your dentist or oral surgeon.
      • Pain management: You may be able to manage pain with an over-the-counter pain reliever, such as acetaminophen (Tylenol, others), or a prescription pain medication from your dentist or oral surgeon. Prescription pain medication may be especially helpful if bone has been removed during the procedure. Holding a cold pack against your jaw also may relieve pain.
      • Swelling and bruising: Use an ice pack as directed by your dentist or surgeon. Any swelling of your cheeks usually improves in two or three days. Bruising may take several more days to resolve.
      • Activity: After your surgery, plan to rest for the remainder of the day. Resume normal activities the next day, but for at least a week, avoid strenuous activity that might result in losing the blood clot from the socket.
      • Beverages: Drink lots of water after the surgery. Don’t drink alcoholic, caffeinated, carbonated or hot beverages in the first 24 hours. Don’t drink with a straw for at least a week because the sucking action can dislodge the blood clot from the socket.
      • Food: Eat only soft foods, such as yogurt or applesauce, for the first 24 hours. Start eating semisoft foods when you can tolerate them. Avoid hard, chewy, hot or spicy foods that might get stuck in the socket or irritate the wound.
      • Cleaning your mouth: Don’t brush your teeth, rinse your mouth, spit or use mouthwash during the first 24 hours after surgery. Typically you’ll be told to resume brushing your teeth after the first 24 hours. Be particularly gentle near the surgical wound when brushing and gently rinse your mouth with warm salt water every two hours and after meals for a week.
      • Tobacco use: If you smoke, don’t do so for at least 72 hours after surgery — and wait longer than that if possible. If you chew tobacco, don’t use it for at least a week. Using tobacco products after oral surgery can delay healing and increase the risk of complications.
      • Stitches: You may have stitches that dissolve within a few weeks or no stitches at all. If your stitches need to be removed, schedule an appointment to have them taken out.

        Apical Resection

        Also known as Apicoectomy, Apical Resection is a surgical procedure to remove the apex or the tip of the tooth’s root. This type of procedure is typically suggested only after a root canal has been performed, and the tooth is still causing you problems.

        Reasons for an Apicoectomy

        As many of us know, infected or inflamed tissue around the root of a tooth can be extremely painful and sometimes debilitating. In the rare instance that a conventional root canal treatment has been performed, but is unable to save your tooth, your endodontist may suggest an Apicoectomy. This will allow for the tooth’s preservation while eliminating any infection. Other reasons why an Apicoectomy may be recommended:

          • Small adjoining root branches: Roots are complex and can contain many tiny branches. If these tiny branches cannot be cleaned and sealed when the root canal treatment is performed, inflammation can persist.
          • Blocked root canal: In some cases, the surgeon is unable to effectively clean a root canal because it is blocked by a fractured file left behind from prior root canal treatment. Infection and debris can quickly affect adjacent teeth.
          • Narrow or curved root canals: When the root canal is poorly shaped, the endodontic files cannot reach the root tip. Continuing infection or re-infection can occur.

        What kind of fillings are available?

        Under local anesthesia, an incision is made in the gum to expose the inflamed tissue and root. The problem area will then be removed using ultrasonic instruments. The root will then be sealed with a small filling and closed up with stitches.

        Although this is a day surgery, the aftercare is crucial to proper healing. Patients are sent home with instructions and pain medication. Discomfort and swelling are normal after surgery. Your connective tissues will fully heal over the next few months.

        1.Is an Apicoectomy the same as a Re-Root Canal?

        A Re-Root Canal is a procedure which is carried out when the initial Root Canal treatment has been unsuccessful, or in the case of recurring infection in the tooth. Re-treatment may be necessary if decay accumulates in the affected tooth, there is a new infection, or if the tooth later cracks or becomes loose.

        An Apicoectomy is different from a Re-Root Canal, as it is a procedure which removes the apex of the tooth, rather than the Root Canal procedure which cleans out infected pulp and fills in the Root Canals afterwards in order to restore the tooth. An Apicoectomy is more complex in nature than a Root Canal procedure.

        The procedure is pain free and done under anaesthesia, so you shouldn’t feel any pain. The procedure is very straightforward and is routine in an endodontic practice. After the procedure, you may experience some pain and swelling, and in some cases, bruising around the area. If you’re in pain, you can take over-the-counter painkillers to combat this.

        After the procedure, you’ll need to eat softer foods and avoid anything which is particularly chewy or crunchy. Brush your teeth carefully as you will have to avoid the area until stitches are removed or fall out on their own. A follow-up appointment will be arranged, to check your progress and to see if the healing is well underway.

        A year or so later, a final X-ray needs to be carried out to check how the bone has healed and if the procedure was successful. Apicoectomy has a high success rate, in the region of 80-95%, so it tends to be a one-off, straightforward procedure with positive outcomes for the patient.

          What is a Sinus Lift?

          A sinus lift, also called sinus augmentation, is a procedure that makes it easier to place dental implants.           This procedure increases the amount of bone in the upper jaw that is required for dental implants. This is one of the most common bone grafting procedures for dental implant patients.

          Who needs a Sinus Augmentation?

          Over the last 25 years, dental implants have become more readily available, making them an increasingly common dental procedure. As more people get dental implants to replace missing teeth, sinus lifts also have become more common.

          A sinus augmentation may be necessary if you are missing teeth and jawbone in the back of your jaw, near your sinuses. Jawbone and teeth in this area could be lost due to birth defects, periodontal disease, cancer, other medical conditions, or just the natural healing process. A sinus lift can also be used if your sinuses are too close to your upper jaw and you need more space to place the implant.

          The Procedure

          The sinus lift procedure is a safe and straightforward procedure. and is generally performed at the prosthodontist’s or oral surgeon’s office. The entire process takes between 90 minutes to two hours.

          • First, the patient is sedated using local and/or general anesthesia. If necessary, a doctor may use nitrous oxide, but this is less common.
          • Next, the surgeon will cut the gum tissue where your back teeth used to be to expose the jawbone.
          • With the tissue raised, and the jawbone exposed, the surgeon will cut a small circle in the bone through which they can gently push on the membrane of the sinuses.
          • As the bony piece is lifted into the sinus cavity space the hole is filled with bone graft.
          • After this is done, the procedure is complete and the surgeon will close the incision with sutures.
          1.What Happens After a Sinus Augmentation?

          Patients are able to go home after they wake from the anesthesia. After the sinus lift procedure, most patients experience a little swelling and bleeding, but only minimal discomfort. The most important part of recovery is to keep the bone graft in place, so it’s important to avoid blowing your nose (or sneezing, if possible). Either of these things could cause the bone-graft material to move and loosen the stitches.

          When you go home, you will likely get a saline spray to keep your nose moist, a prescription to prevent congestion and swelling, a prescription for pain medicine, an antibiotic, and/or an antimicrobial mouthwash to help prevent infection. Most patients will be in bed with ice around the area for a day or so until they can resume normal activities.

          You will have a follow-up appointment with the dental specialist, during which, the specialist will examine the surgical site and may remove any stitches that have not dissolved on their own.

          A full recovery from a sinus lift takes a minimum of 6 months for the sinuses to heal and the bone to be strong enough to support dental implants. Although the process may seem slow, you are working towards results that will last a lifetime.

          Dental fillings are a very good way to repair tooth cavities, and they can stop toothache while preventing further tooth decay. But it’s much better to avoid tooth fillings in the first place. You can help to do this, with advice from a dental hygienist or dentist, by:

          • Avoiding/cutting down and managing your intake of sugary or acidic foods and drinks. If you sip on a sugary drink throughout the day, for example, your teeth are exposed to the sugar for longer, and your mouth doesn’t have chance to recover
          • Seeing a dentist and/or dental hygienist regularly
          • Cleaning between your teeth before brushing
          • Brushing your teeth twice a day for two minutes
          • Using the right toothpaste; not all types of toothpaste are the same, and different types are better suited for different needs depending on your age and a variety of other factors

          A sinus lift is a surgical procedure and therefore carries some inherent risks. The main risk of this surgery is the piercing or tearing of the membrane.             All on 8 Dental Implants

            An All-on-8 implant system is a good option for patients living with edentulism (toothlessness), as it can be implemented for upper and lower restorations.             The Procedure

              All-on-6 Dental Implants

              Not having a full set of teeth can make you self-conscious and diminish the wattage of your smile. There’s also the more serious issues of bone loss, tooth decay and poor oral hygiene. The All-on-6 technique offers a permanent solution and restores your full mouth of teeth.

              How do All-on-6 implants work?

              First let’s review what a dental implant is. A dental implant is a small titanium screw, which is placed into your jawbone to replace the root of a missing tooth. Once the implant is in place, a crown is attached to give you a highly realistic looking and functional tooth.

              Because the implants are made of titanium, they can fuse to the jaw bone in a process called osseointegration. This makes them incredibly sturdy and reliable, just like real teeth.

              All-on-6 implants are the perfect solution for patients that are missing all of their teeth in either their upper and/or lower arches.

              With the All-on-6 a series of 6 implants are placed to anchor a whole upper or lower arch of teeth. Instead of getting a dental implant for each missing tooth, we are able to strategically place just 6 implants.

              These full-mouth restorations are non-removable, incredibly stable, and function just like real teeth. With All-on-6 implants, you can chew tough foods, eat fresh fruits and vegetables, laugh, kiss, and speak confidently again.

              They also provide the added benefit of preserving your jawbone. When teeth are lost, the jawbone below your teeth slowly starts to recede. By placing a titanium implant into the jawbone, you will prevent further bone loss and facial sagging, creating a more youthful appearance.

              Who can have the All-on-6 Dental Implant treatment?

              Before the All-on-6 dental implants procedure our dentists will make an X-Ray and examine your mouth. They will also discuss your medical history as systemic diseases can affect your healing.

              If you have experienced bone loss for whatever reason it may be, an All-on-6 dental implant treatment might still be possible as our dentists can build up bone for implant levels using sophisticated bone grafting techniques.

              Though every patient case is unique, our dentist are looking at five major factors to consider when recommending the All-on-6 dental implant treatment:

              • Speech Issues: If speech is a major concern, periodontists must be prepared to offer alternative treatments, such as to replace missing teeth with short-span bridges or single dental implants. This will often require more implants, more bone grafting, and more expensive prosthetics, so patients will need to be prepared for the additional treatment time and cost.
              • Bridge thickness: This can be a big problem for some patients, and sadly they often figure out that it’s a problem after surgery when the ridge has been reduced. At that point, nothing can be done to get them back to that natural-feeling transition.
              • Excessive biting force: Patients have difficulty recognizing premature or excessive occlusal contacts. They may generate excessive biting forces due to the lack of sensation.
              • Parafunction: Bruxism, clenching, and irregular chewing cycles

              The Procedure

              The process of getting All-on-6 implants is an efficient and less painful way to replace a full arch of missing teeth. Individual dental implants to replace 24 functional tooth is impractical and not recommended. With All-on-6 implants, you can have all the implants placed in one surgery, which will minimize discomfort and recovery time.

              • The first step of placing the implants is making 3D images of the jaw, so that the dentist gets a detailed picture of the condition the jaw bone is in, as well as the position of sinuses and nerves in relation to the jaws. If it is determined that the patient has enough bone tissue to proceed with All-on-6 technique, a precise plan of implant positions is created.
              • Once the initial process has been finished, the teeth indicated for extraction are pulled out, after which the implants will be placed immediately. If there is any sort of bone defect in the jaws region, the specialists make sure to avoid it. In certain rare circumstances it is necessary to wait for a while for the wounds to heal in order to place the implants. All implants in both jaws can be placed in a single day, and they are mostly placed in the position where the 2s, 4s and 6s used to be.
              • After the placement of the implants, there is a period of bone integration (osseintegration), lasting between 3 and 6 months.

              • As mentioned earlier, the final arch, will be placed three to six months after the placement of your implants. The crowns come in two options: metal ceramic or non-metal zirconium. In most cases, a 12 crown arch is planned, i.e. up to the 6s, but this can vary. Only the All-on-6 features six implants and 12 crowns, since it is considered that 12 crowns is optimal for normal daily functions.

              After the placement of implants, patients must maintain scrupulous hygiene around the implants. If they live a healthy lifestyle and keep a good hygiene, a gradual regeneration of bone tissue can be expected. The cost of the All-on-6 implant treatment depends on the type of implants and crowns to be used, and it is paid in two installments. You will be charged for implants and temporary fixtures during your initial visit. As for the placement of your final arch, this part can be paid for during your second visit.

              1.All-on-6 vs. All-on-4

              Both procedures are low-cost and effective, making them an ideal choice. However, some may ask why they should choose an All-on-6 as opposed to the All-on-4? Neither of these options is necessarily better; it is a decision that should be made on a case-by-case basis. For example, if a more extensive reconstruction is needed, All-on-6 implants are a better choice because the additional implants provide additional support. However, if you do not have the additional bone mass required for the additional implants, you would be a more likely candidate for an All-on-4.
